An exciting opportunity to join the REACT (Reactive Emergency Assessment Community Team) as a Healthcare Support Worker, working across Ipswich and East Suffolk to assess and support patients in short-term crisis and help them remain at home where possible. The integrated multi-agency team includes Nurses, Physiotherapists, Occupational Therapists, Doctors, Healthcare Assistants, Social Workers and partner organisations such as Suffolk Family Carers, Citizens Advice and the British Red Cross.
The role is community- and patient-facing, providing one-to-one support in patients' homes using a reablement approach and supporting clinicians by completing delegated tasks to meet individual needs. The REACT service provides 24-hour support; shifts include early, late, night, weekend and bank holiday working.
The successful applicant will be offered either Band 2 or Band 3 depending on previous experience and qualifications, with opportunities for training and progression.
You must hold a valid UK driving licence and be able to travel to patients' homes using your own vehicle or a Trust pool car. £24,465 per year
£24,465 - £26,598 per annum pro rata. Additional benefits include a generous pension scheme, unsocial hours payments (where applicable), 27 days annual leave on commencement (pro rata), access to NHS discounts, and staff health and wellbeing services.
